Classic microneedling (also known as Collagen Induction Therapy) is a minimally invasive procedure that uses ultra-fine, medical-grade sterile needles to create thousands of controlled microscopic punctures in the top layer of the skin.


How Classic Microneedling Works

  • Controlled Micro-Injuries: A motor-driven pen cartridge that glides across the skin, puncturing the epidermis into the papillary dermis ( depths ranging from 0.5 mm to 2.5 mm).

  • Wound Healing Cascade: The micro-channels trigger the body's natural wound repair response without damaging the outer epidermal barrier:

    1. Inflammation Phase: Platelets release growth factors to jumpstart cellular repair.

    2. Proliferation Phase: Dermal fibroblasts synthesize new Type III collagen, elastin, and extracellular matrix.

    3. Remodeling Phase: Fresh collagen matures into structured Type I collagen, thickening and firming the dermal layer.

  • Transdermal Absorption: Opens temporary micro-channels allowing topical serums (such as hyaluronic acid, exosomes, or growth factors) to penetrate directly into the deeper dermal layers.

Radiofrequency (RF) microneedling combines traditional mechanical microneedling with thermal radiofrequency energy, delivering heat directly into the deep dermis to stimulate collagen contraction and tissue remodeling.


Dual-Mechanism Action

  • Targeted Mechanical Micro-Injuries: Micro-needles penetrate the epidermis to puncture the dermis, creating controlled physical channels that trigger the body's natural wound-healing process.

  • Deep Thermal Coagulation: Once the needles reach their target depth (typically 0.5 mm to 4.0 mm), radiofrequency energy emits from the needle tips. This heats dermal tissue to approximately $60^\circ\text{C}$$70^\circ\text{C}$, causing immediate collagen fiber contraction and signaling long-term neocollagenesis.

  • Epidermal Protection: Because the energy bypasses the outermost skin layer (especially when using insulated needles), thermal damage to the surface skin is minimized, making it safe for darker skin tones (Fitzpatrick types IV–VI) that are prone to post-inflammatory hyperpigmentation from surface lasers.
